Hospital issues a form when you come out of hospital or visit service NSW and
they will give the form for applying birth certificate
2.
Please take the snap of “Doctor/Midwife – Proof of declaration” page ( very imp for future
use)
3.
Submit all the document in service NSW

Medicare card

Debit card/credit card
3.
Australian DL
4.
Foreign Passport (indian)
5.
Utility bills with current address

to apply for Citizenship certificate
1) Download the form 119 (Application for evidence of
Australian citizenship) https://www.border.gov.au/Forms/Documents/119.pdf
2) Fill the form and post it via nearest AU post

2.
Once all the fields have been filled , Submit
the application and take the print out of the same

Book an appointment at the local AU post office
via Australian Post

The guarantor must:
. be an Australian citizen who is 18 years
of age or over
. have known the applicant for at least 12
months (or, for children under one year, since birth)
. not be related to the applicant by birth,
marriage, de facto or same sex relationship, or live at the applicant's address
. possess a current (unexpired) Australian
passport that was issued with at least two years validity or have been on the
Australian electoral roll at their current address for the past 12 months;
. endorse the back of one photo by writing
'This is a true photo of (applicant's full name)' and signing in black pen.

Procedure
to apply for OCI for Kids
1.
Go to OCI VFS website and read the contents
2.
Click Next button at the bottom
3.
Step1 – Your Residential State information
4.
Step2 – Download all the forms as applicable
Note: for my baby only “Form U” was applicable along with “Overseas Citizen of India (OCI) Checklist”
a. Complete “online application form” link
b. It will take us to “Online OCI
Services”
c. Click on “Proceed” button at the bottom
d. Select “ New Registration” option
And follow the prompt to complete Section A, Section B and Document
upload
e. Once Online application is completed, take
the Print out of application

Step3 – Book an Appointment at VFS and submit
all the documents including the online application

1. Both parents Current Passport Bio Page copy

Indian Origin ( Atleast minimum 3 document below is required)
